•  Example Emailed statement summary

•  Example Mailed hard copy bill

2.  Type your name as it appears on your bill or the company name.  Do not use both.

3.  If you have telephone service from InfoStructure, use one of your telephone numbers to register.  If you do not have telephone service from InfoStructure, use all ZEROS for the telephone number when registering.
